Extra Care and Support for People With Low Incomes or Certain Medical Conditions

Special Needs Plans are Medicare Advantage plans for people living with certain conditions or diseases, or those with low incomes. It can be hard to have a condition that requires extra health care and just as difficult to cover health costs on a fixed or low income. We’re here to support you with extra benefits, drug lists and care networks that fit your special needs.

Take Advantage of Extra SNP Coverage

If you have both Medicare and Medicaid, our Dual-Eligible Special Needs Plans (D-SNPs) include dental, hearing, vision and prescription drug coverage. You don’t need to pay anything for a routine teeth cleaning or eye exam. Many of our plans also cover hearing aids.

Our Medicare Advantage SNPs include Medicare Part A and Part B, but they also cover what Original Medicare does not. There are benefits for routine care, and resources like money for health-related items.

Types of Special Needs Plans – D-SNP, C-SNP and I-SNP

Dual-Eligible Special Needs Plan and Eligibility

You’re eligible for a D-SNP if you have both federal Medicare and state-run Medicaid. 

To be eligible for Medicare, you need to be 65 or older or have certain health conditions. Medicaid, on the other hand, is based on income levels instead of age. If your income qualifies you for assistance, you can be younger than 65.

Chronic Condition Special Needs Plan and Eligibility

We have Chronic Condition Special Needs Plans (C-SNPs) for people living with diabetes, a cardiovascular or lung disorder, chronic heart failure, or end-stage renal disease (ESRD). If you have one or more of these conditions and live in an area we cover, you can enroll in a C-SNP for benefits catered to your specific needs.

Most of our Medicare Advantage C-SNPs include prescription drug coverage, as well as $0 monthly premiums, $0 deductibles and $0 copays. Learn more about our C-SNPs and the benefits that come with them.

We also have Institutional SNPs (I-SNPs) for people living in one of our network nursing homes or assisted living communities, or other plan-approved locations.
