Anthem Chronic Condition Special Needs Plans (C-SNPs)

If you're living with a special condition such as diabetes, end-stage renal disease, lung disorders or heart disease, access to the right health care and doctors is vital. We have Medicare Advantage plans to cover extra care and daily needs that help you stay your healthiest. It’s true that living with a chronic condition is difficult, but a customized health plan and resources can make life easier.

For example, C-SNPs with diabetes coverage often include supplies to monitor blood glucose and insulin. These plans also connect you with specialists and a care team to support your health needs.

Am I Eligible for a C-SNP? What Conditions Do They Cover?

If you’re living with one or more of the following conditions and we offer the plan where you live, you may be eligible. Coverage varies depending on where you live, so check your area for specifics. Exploring the extra benefits you could have is worth it – it could even be life-changing.

Diabetes mellitus:

C-SNPs with diabetes coverage typically include blood glucose and insulin management tools, as well as vision, hearing, dental and foot care.

They also offer nutritional training and extra screening and monitoring. With a C-SNP Medicare plan, your doctor and an endocrinologist (diabetes specialist) will work with you to determine the most effective care.

We currently offer C-SNPs with diabetes coverage in Colorado, Nevada and Virginia.

 

End-stage renal disease (ESRD):

If you have this kidney condition, C-SNPs offer care teams and nurses who work with your primary doctor and nephrologist (kidney-care specialist).

You’ll receive personal support, nutritional training and the necessary treatments, including prescription drug coverage. Routine and preventive care, such as health screenings, dental, vision and hearing, is also covered.

We currently offer C-SNPs with ESRD coverage in Connecticut.

Chronic lung disorders:

These C-SNPs offer special care for those with lung-related disorders such as chronic obstructive pulmonary disease (COPD), asthma, chronic bronchitis, pulmonary hypertension or emphysema.

These plans include special coverage for items like oxygen supplies and other medical equipment. A pulmonologist (lung specialist) will work with your doctor for treatment, medications and health management.

We currently offer C-SNPs with coverage for chronic lung disorders in Nevada and Virginia.

 

Chronic heart failure and cardiovascular disorders:

This C-SNP has special care for you, if you have chronic venous thromboembolic disorder, chronic heart failure, cardiac arrhythmias, coronary artery disease, or peripheral vascular disease.

A cardiologist (heart specialist) will work with you and your doctor to determine the best course of ongoing care.

We currently offer C-SNPs with coverage for chronic heart failure and cardiovascular disorders in Nevada.

What Benefits and Coverage Come With C-SNPs?

Most of our Medicare Advantage C-SNPs include prescription drug coverage, as well as $0 monthly premiums, $0 deductibles and $0 copays. Plus, if you’re living with more than one chronic condition, some C-SNPs have supplemental coverage too. For example, a C-SNP that covers cardiovascular disorders may include coverage for diabetes care also.
